Patient Grievances
If patients have complaints or concerns in regard to your care at the Surgical Center for Excellence, they are encouraged to provide feedback via our patient survey in person or online. You may also contact the center’s administrator through our website or via phone at 850-522-1930. If you would like to file a Grievance that cannot be handled by the center’s administrative staff you may utilize one of the methods below:

Advance Directives
If you have advance directive or living will, The Surgical Center for Excellence, will not honor any living will that does not allow life-saving resuscitation. In the event of a medical emergency, the center will also transfer you to the nearest hospital for acute care services, which will make decisions about following any advance directive or living will. You have the right to have an advanced directive or living will on file with the center; we will forward to the receiving hospital with your transfer. Information and forms can be found online:
